About
The Early Menopause Assessment Panel is a blood test that measures six hormones involved in ovarian function, menstrual regulation, ovulation, prolactin balance, and thyroid function. It includes follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol, progesterone, prolactin, and thyroid-stimulating hormone (TSH).
The panel may support the evaluation of absent or irregular periods, changes in menstrual frequency, hot flashes, night sweats, sleep disturbance, vaginal dryness, reduced fertility, or other symptoms occurring earlier than expected. In women under 40, persistent menstrual disturbance and compatible laboratory findings may raise concern for premature ovarian insufficiency. Menopause occurring between the ages of 40 and 45 is generally described as early menopause.
Symptoms that may lead to hormone testing
Hormonal changes may be considered when menstrual changes occur together with fatigue and low energy, poor concentration and brain fog, weight gain and difficulty losing weight, or headaches, migraines and dizziness. These symptoms are nonspecific and can also be connected with thyroid disorders, anemia, sleep problems, stress, medication effects, pregnancy, or other medical conditions. The panel helps provide laboratory context but does not determine the cause of symptoms by itself.
What the Early Menopause Assessment Panel measures
- FSH: Helps regulate follicle development in the ovaries. Persistently elevated levels may be associated with reduced ovarian activity, but interpretation depends on age, menstrual history, and other hormone results.
- LH: Participates in ovulation and ovarian hormone production. It is interpreted together with FSH and estradiol.
- Estradiol: The main circulating estrogen during the reproductive years. Lower levels may support evidence of reduced ovarian estrogen production when assessed in the appropriate clinical context.
- Progesterone: Changes considerably during the menstrual cycle and may provide information about ovulation and cycle phase.
- Prolactin: Elevated prolactin can contribute to irregular or absent periods and may indicate another hormonal explanation for menstrual changes.
- TSH: Assesses thyroid regulation because thyroid dysfunction can cause menstrual changes, fatigue, weight changes, and concentration problems that may resemble menopausal symptoms.
How the testing pathway works
Step 1: Review the menstrual pattern
Before testing, it is useful to record when the last menstrual period occurred, whether cycles have become irregular, how long changes have been present, and whether hormonal contraception or hormone therapy is being used. Previous hormone results, gynecologic reports, fertility records, and relevant medical history may also help with interpretation.
Step 2: Schedule the blood sample
When menstrual cycles are still present, the blood sample is taken on day 2, 3, or 4 of the cycle. The first day of menstrual bleeding is counted as day 1. Patients without menstrual periods should follow the timing instructions provided when arranging the test.
Step 3: Blood collection and laboratory analysis
A venous blood sample is collected and analyzed for FSH, LH, estradiol, progesterone, prolactin, and TSH. The procedure does not involve hormone administration or any other treatment.
Step 4: Medical interpretation
The results should be interpreted together rather than as six isolated values. The menstrual cycle phase, age, symptoms, medications, hormonal therapy, pregnancy status, and the laboratory reference ranges can all affect their meaning.
Why one hormone result may not confirm early menopause
FSH and other reproductive hormones can fluctuate, particularly when ovarian function is changing but has not stopped completely. One elevated or otherwise abnormal result therefore does not necessarily confirm early menopause or premature ovarian insufficiency. When the clinical picture and laboratory result do not clearly match, FSH may be measured again after approximately 4 to 6 weeks.
Age and the menstrual pattern remain essential. A result from a woman with several months of absent periods is interpreted differently from the same value in a woman with regular cycles. A gynecologic assessment may include a detailed history, examination, pregnancy testing, pelvic ultrasound, additional ovarian reserve testing, or investigations for autoimmune, genetic, metabolic, or treatment-related causes when clinically appropriate.
What the results can and cannot show
The panel can identify a hormone pattern that supports further evaluation of ovarian function and can help reveal thyroid or prolactin abnormalities that may contribute to menstrual changes. It cannot independently establish the cause, predict the exact timing of the final menstrual period, measure fertility potential with certainty, or replace a gynecologic evaluation.

Candidate
The panel may be considered for women experiencing absent, irregular, or noticeably changed menstrual cycles at an earlier age than expected, particularly when changes occur with hot flashes, night sweats, sleep disturbance, vaginal dryness, fertility concerns, or other possible hormonal symptoms. It may also support the evaluation of suspected premature ovarian insufficiency in women under 40. The panel is not a standalone screening test for every nonspecific symptom. A clinician should determine whether hormone testing, pregnancy testing, gynecologic assessment, or another diagnostic pathway is more appropriate.
Preparation
If menstrual cycles are present, the blood sample should be scheduled on day 2, 3, or 4 of the cycle, with the first day of menstrual bleeding counted as day 1. Record the date of the last menstrual period and any recent changes in cycle length or frequency. Provide information about hormonal contraception, hormone therapy, fertility medication, other regular medication, pregnancy or breastfeeding, and previous hormone results. Prescribed medication should not be stopped unless a physician gives specific instructions.
Treatment
A venous blood sample is collected and analyzed for FSH, LH, estradiol, progesterone, prolactin, and TSH. No hormones or medications are administered as part of the test. The measured values are reported with the laboratory reference intervals for subsequent medical interpretation.
Result
The report provides individual values for all six hormones. Their combined pattern may support evaluation of ovarian activity, menstrual regulation, ovulation, prolactin balance, and thyroid function. A single result may not confirm early menopause or premature ovarian insufficiency. Interpretation should consider age, symptoms, menstrual history, cycle day, medication use, and previous findings. FSH or other testing may be repeated when diagnostic uncertainty remains, and gynecologic evaluation may be recommended.
Precautions
Hormone values can vary with the menstrual cycle and may be influenced by hormonal contraception, hormone therapy, fertility treatment, pregnancy, breastfeeding, certain medications, illness, and other endocrine conditions. Results should not be interpreted solely by comparing one value with a reference range. The panel does not replace pregnancy testing, pelvic examination, ultrasound, fertility assessment, or other investigations when these are clinically indicated. New, persistent, or worsening menstrual changes should be discussed with a gynecologist or another appropriate physician.
