The Prostate Health Panel is a combined laboratory assessment that measures total prostate-specific antigen (PSA), free PSA (fPSA), and selected urine parameters. It is intended to provide initial information about prostate health while also checking the urine for findings that may be associated with inflammation, infection, bleeding, or other urinary tract changes.

The panel can support preventive assessment, follow-up of previously measured PSA values, or further evaluation when a man has urinary symptoms. It does not diagnose prostate cancer, benign prostate enlargement, prostatitis, or a urinary tract infection on its own. The findings must be interpreted together with age, previous PSA results, symptoms, family history, medications, recent procedures, and a urological examination when indicated.

What the Prostate Health Panel includes

  • Total PSA: measures the overall amount of prostate-specific antigen circulating in the blood.
  • Free PSA: measures the fraction of PSA that circulates without being bound to other proteins.
  • Urine dipstick: provides a chemical assessment of urine parameters that may include blood, leukocytes, nitrites, protein, glucose, pH, and specific gravity.
  • Urine sediment: uses microscopic examination to look for cells, bacteria, crystals, casts, and other elements in the urine.

Total PSA and free PSA are measured from the same serum sample. The relationship between free and total PSA may provide additional context when evaluating certain PSA results. More information about the individual blood marker is available on the PSA screening page.

When the panel may be considered

The panel may be considered by men who want an initial laboratory assessment of prostate health, particularly after the age of 45. It may also be relevant for men with a family history of prostate disease, previous PSA measurements that require comparison, or urinary symptoms such as frequent urination, urination during the night, difficulty starting urination, a weak or interrupted urine stream, urgency, discomfort while urinating, or a feeling that the bladder has not emptied completely.

Men without symptoms should discuss the potential benefits and limitations of PSA-based testing with a doctor. The appropriate time to begin testing and the interval between tests depend on individual risk, age, general health, family history, previous findings, and personal preferences. A single universal testing schedule is not suitable for every man.

How the panel is performed

Step 1: Preparation before sample collection

For accurate PSA and fPSA measurement, mechanical irritation of the prostate should be avoided during the 72 hours before blood collection. This includes procedures or examinations that directly affect the prostate. Intensive physical activity should also be avoided during this period, particularly cycling, because it may temporarily influence the PSA concentration.

The urine portion requires the midstream of the first morning urine. The sample should be collected in a sterile container and delivered for analysis as soon as possible. Following the collection instructions helps reduce contamination and improves the reliability of the urine findings.

Step 2: Blood collection

A blood sample is taken from a vein. Serum from the same blood sample is used to measure both total PSA and free PSA, allowing the two results to be considered together.

Step 3: Urine collection

The first small amount of urine is passed into the toilet, after which the midstream portion is collected in the sterile container. The urine is assessed using a test strip and microscopic sediment examination.

Step 4: Laboratory analysis and report

The report includes the measured total PSA and free PSA values together with the urine dipstick and sediment findings. Results for this panel are available on the same day.

How PSA results should be interpreted

PSA is produced by prostate tissue, but it is not specific to prostate cancer. An elevated result may occur with benign prostate enlargement, inflammation of the prostate, urinary infection, recent mechanical irritation of the prostate, recent prostate procedures, or other noncancerous changes. Age can also influence the expected PSA level.

For this reason, an elevated PSA result does not confirm cancer. A PSA result within the laboratory reference range also cannot completely exclude it. The result is more informative when compared with previous measurements and interpreted in relation to age, symptoms, family history, clinical findings, and the urologist’s assessment.

Free PSA provides additional information because the proportion of free PSA can differ between benign prostate conditions and prostate cancer. However, neither free PSA nor the free-to-total PSA relationship establishes a diagnosis. These findings may help a doctor decide whether repeating the test, clinical monitoring, a urological examination, imaging, or another diagnostic step is appropriate.

What the urine analysis adds

The urine dipstick and sediment examination can identify findings such as leukocytes, erythrocytes, nitrites, bacteria, crystals, protein, or other elements. These findings may support the assessment of urinary symptoms and may suggest inflammation, infection, bleeding, stone-related changes, or another urinary tract issue.

A routine urine examination is not the same as a urine culture. If bacteria or other findings suggesting infection are present, or if urinary symptoms continue despite an unremarkable result, a urine culture or further medical evaluation may be recommended.

What may happen after the results

No additional procedure is automatically required after the panel. The next step depends on the combination of PSA, free PSA, urine findings, symptoms, age, previous values, and clinical risk.

A doctor may recommend repeating PSA under standardized conditions if the result may have been temporarily affected. Persistent or clearly elevated values may lead to a urological examination, digital rectal examination, prostate imaging, or other risk assessment. Abnormal urine findings may require a urine culture, kidney and urinary tract evaluation, or treatment based on the identified cause.

The panel may be suitable for men who want an initial laboratory assessment of prostate and urinary tract health, particularly those over 45, men with a family history of prostate disease, men with previous PSA results that require monitoring, and those with urinary symptoms such as frequent or nighttime urination, difficulty starting urination, a weak stream, urgency, or incomplete bladder emptying. PSA-based testing in men without symptoms should be considered individually according to age, health, risk factors, previous values, and medical advice.

Preparation

Avoid mechanical irritation of the prostate and intensive physical activity during the 72 hours before blood collection. Activities such as cycling may temporarily influence PSA values. Collect the midstream of the first morning urine in a sterile container and deliver it for analysis as soon as possible. Inform the medical professional about recent prostate examinations, biopsies, procedures, urinary infections, current medications, and previous PSA results.

Treatment

A venous blood sample is collected for total PSA and free PSA measurement, and a urine sample is provided for dipstick testing and microscopic sediment examination. Total PSA and free PSA are measured from the same serum sample. The urine sample should consist of the midstream portion of the first morning urine collected in a sterile container.

Result

The report includes total PSA, free PSA, and urine dipstick and sediment findings, and is available on the same day. PSA results must be interpreted according to age, previous measurements, symptoms, family history, recent procedures, and urological findings. An elevated PSA does not confirm prostate cancer, while a result within the reference range does not completely exclude it. Additional evaluation may include repeat testing, a urological examination, imaging, or urine culture, depending on the findings.

Precautions

PSA is not specific to prostate cancer. Benign prostate enlargement, prostatitis, urinary infection, recent mechanical irritation, prostate procedures, and intensive cycling or physical activity may increase the result. Free PSA provides additional context but is not diagnostic on its own. Visible blood in the urine, inability to urinate, fever with urinary symptoms, severe pelvic or back pain, or rapidly worsening symptoms require prompt medical assessment rather than relying only on a laboratory panel.
